Medication Errors – How to Stop Them?

Proper quality management focuses on managing "processes."  Another area of the healthcare industry grapples with this.  What do you think it would take to get our industry to stop counting and scolding MTs about their errors and, instead, put processes in place the find and correct them?  Telling people to stop making errors is an exercise in insanity.  Proofreading our own work is insanity.  Docking human beings for making errors is abusive.
